了吗 asks whether something has happened yet.

How it works
你吃饭了吗? is "have you eaten?". The 了 marks completion and 吗 makes it a question.
The negative answer drops the 了: 还没吃 or 没有. Answering 没吃了 is wrong.
了没有 is a common spoken alternative with the same meaning: 你吃饭了没有?
